Registration and Withdrawal

REGISTRATION & WITHDRAWAL

Registration and withdrawal of a student can be a lengthy process, so please plan ahead and make an appointment or call to ensure the counselor will be available for these services.  24 hours notice is kindly requested to process a student's withdrawal. 

NEW STUDENT REGISTRATION:

Parents must accompany students to enroll and provide the following items at the time of registration: Please do not schedule an appointment to enroll until all items are secure.  We are unable to schedule a student until all items have been cleared.   

  • Parent/Guardian identification (must match birth certificate) - Photo ID required
  • Alabama immunization form - Out of state forms cannot be accepted.  Please call the  Coffee County Health Department or Lyster Army Hospital for instructions on obtaining an original immunization card.
  • Proof of residency in legal guardian's name - 2 current items are required. (1st Item:  Lease or Mortgage Statement / 2nd Item:  Utility bill are acceptable forms of residency verification)   If residency documentations are not in legal guardian's name, additional clearance must be obtained from the central office prior to registration.
  • Copy of birth certificate - for age and custody verification
  • Withdrawal form or most recent report card from former school to verify grade and course placement (cannot receive a schedule without this).  Please ask the previous school to provide at least a withdrawal form until official records can be requested.
  • Copy of IEP or 504  if applicable
  • Custody documentation if applicable (must be signed by a judge)
